*A note regarding end of school medication pick up
The last day to pick up any prescription or over the counter medication kept in the clinic will be Thursday, May 27th. Please make arrangements to have the medication picked up by 10 a.m. on that day. Please come to the North Clinic doors (red doors with numbers 7600 above it) and let us know you are here by calling 205-228-3228 or 205-228-3218. We will bring the medication to your car. Any medication not picked up by Thursday at 10 a.m. will be discarded. Please be aware, we are not able to send any medications home with students. Students are not allowed to transport medication to or from school. A parent or legal guardian must be the one to pick medication up. The clinic is not allowed to store any medication over the summer.
Summer is a great time to get medication forms completed for the next school year and update Immunization Forms. If your child will require medication at school during the 2021-2022 school year, medication forms are available on the Trussville City Schools website and in our front office here at Paine. Most local doctor offices have the forms also.

*Attention parents of students needing to take medication at school!*
Below is the Alabama State Department of Education School Medication Prescriber/Parent Authorization Form. This form MUST be filled out by your doctor before any prescription medication can be given to your children at school. Any changes in dosage or time given require new authorization forms from the doctor.
This form is also required for over the counter medications but a physician signature is not required for these.
