Attention Parents
Any medications at school will need to be picked from the clinic up on or before May 25th by 10:00 am. Any medications not picked up by this time will be discarded. Please make arrangements to have medications picked up by a parent or guardian, we are unable to send medication home with students.
If your child will require medication at school during the 2022-2023 school year, medication forms are available on the Trussville City Schools website, using the link provided below or in our school's main office. Most local health clinics also provide these forms.

Below is the Alabama State Department of Education School Medication Prescriber/Parent Authorization Form. This form MUST be filled out by your doctor before any prescription medication can be given to your children at school. Any changes in dosage or time given require new authorization forms from the doctor.
This form is also required for over the counter medications but a physician signature is not required for these.
